If you are returning participant of the Spring Tryouts, no additional forms will be needed for tryouts unless a new injury has occurred. If so, please have a new physical form completed providing your release to participate.
ALL NEW PARTICIPANTS, PLEASE COMPLETE THE TRYOUT APPLICATION AND RETURN BY AUGUST 18, 2009
TRYOUT SCHEDULE
Friday, August 21
PLEASE REPORT FOR REGISTRATION AT 4:00 PM IN THE USC FIELDHOUSE
4:00-9:00 PM
Saturday, August 22
10:00AM-1:00 PM
Sunday, August 23 (practice with new team members)
2:00 PM-5:00 PM
Please bring the following with you to tryouts:
- A copy of acceptance letter to USC
- The completed insurance form
- The signed waiver
- The completed physical form
